IN CONSIDERATION OF PLAYER ABOVE my minor child/ward ('my child'), being allowed to participate in any way in the RUSH FIELD HOCKEY INC. program, related events and activities, the undersigned acknowledges, appreciates, and agrees that:

1) The risk of injury to my child from the activities involved in these programs is significant, including the potential for permanent disability and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,

2) FOR MYSELF, SPOUSE, AND CHILD, I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASES or others, and assume full responsibility for my child's participation; and

3) I willingly agree to comply with the program's stated and customary terms and conditions for participation. If I observe any unusual significant concern in my child's readiness for participation and/or in the program itself, I will remove my child from the participation and bring such attention of the nearest official immediately; and,

4) I myself, my spouse, my child, and on behalf of my/our heirs, assigns, personal representatives and next of kin, HEREBY RELEASE THE other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event ("Releasees"), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property incident to my child's involvement or participation in these programs, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

5) I, for myself, my spouse, my child, and on behalf of my/our heirs, assigns, personal representatives and next of kin, HEREBY INDEMNIFY AND HOLD HARMLESS all of the above Releasees, RUSH, and/or either of their affiliated clubs, their respective administrators, directors, agents, coaches, sponsors, advertisers, officers, volunteers and if applicable owners and leasers of premises used to conduct the event from any and all liabilities incident to my involvement or participation in these programs, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent permitted by law.

6) I hereby grant permission to RUSH to use my photograph, image or likeness in any or all official RUSH publications, film, video or RUSH website(s) without further consideration or compensation. I understand that RUSH retains ownership of all photographs and images and any and all rights to the photographs images in any format or medium.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUEMENT.

Q: Do I have to be a member to participate in RUSH® programs?

A: Yes, RUSH® membership is required before registering for RUSH programs.

Q: How do I become a RUSH® member?

A: Click on Membership and fill out the form to become a Rush® Member.

Q: What does it cost to be a RUSH® member?

A: The cost of membership is $35.00 per year.

Q: Is my membership fee refundable or transferable?

A: All RUSH® Field Hockey memberships are non-refundable. Memberships are not transferable.

Q: How long is my membership valid for?

A: Memberships are valid for all of 2010. Participants must renew membership once a year.

Q: How will I know if I registered to be a member correctly?

A: Once successfully signed up for a membership, you will receive a confirmation email with your username and password. You will then be able to log in to the site and registering for future programs.

Q: What if I can’t remember my username & password?

A: You can reset your password here.

Q: What if there is an error or change in my membership information. How can it be corrected and/or updated?

A: You can edit & update your personal information here.

Q: When signing up and registering for a new program, will I need to fill out a registration form?

A: No, once you become a RUSH member you will no longer need to fill out registration forms. All that is needed to register for a new programs is click on the desired program, type in your assigned username and password and pay the associated fee.
